FOR NEW APPLICANTS:

Step 1. Register to the website www.btofms.com (Only for unregistered member only), for registered member proceed to Step 2.
a. Fill-up the account registration form (Also provide your personal email).
b. Verify your email.
c. Done. Note: that the account you created will be blocked or deleted in 24h. If you did not proceed to apply or renew Franchise after registering an account.

STEPS ON HOW TO APPLY FRANCHISE:

Step 2. Login to your account.
Step 3. Click the “Apply” button.
Step 4. Fill-up the application and upload the required documents.
Step 5. Submit the application form.
Step 6. Check your status of application.
Step 7. If status of the application is “For Inspection”, bring your vehicle at the MTFRU office for the Inspection.
Step 8. If status of the application is “For Payment”. Pay the necessary fees at the accounting, and get your receipt of payment. And hand your receipt to the Clerk at the office of MTFRU for proof of payment.

Medical Form
• Photocopy of Original Receipt (O.R) and Certificate of Registration (C.R)
• Community Tax Card (Cedula)
• Barangay Clearance
• Voter's Certificate
• Police Clearance
• Photocopy of Driver's License
• Photocopy of Values Formation Seminar Certificate

FOR RENEWAL OF FRANCHISE:

• Medical Form
• Photocopy of Original Receipt (O.R) and Certificate of Registration (C.R)
• Community Tax Card (Cedula)
• Barangay Clearance
• Voter's Certificate
• Photocopy of Driver's License
• Photocopy of Values Formation Seminar Certificate

GUIDELINES:

• If a Franchise Owner accumulates more than three violations, their driver's license may be confiscated, their Bajaj tricycle could be impounded, or their franchise might be suspended.
• When updating your information, only your contact number and email will be modified for verification purposes.
